As the participant, legal parent or guardian, (referred to as “Participant”), I release and hold harmless Boogaloo Academy Ltd./A Star Society, its personnel and agents, including but not limited to, executives, organizing committees, contractors, employees and volunteers (individually and collectively, "Boogaloo/A Star") from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the Participant and/or the undersigned, while: participating in the courses, (including online classes), rehearsals, recitals, performances, and events (individually and collectively, "Events") ") presented or organized by or under the auspices of Boogaloo Academy Ltd./A Star Society; upon the studio premises or any premises , including taking virtual classes at your own home, under the control and/or supervision of Boogaloo/A Star; enroute to or from any of said Events or premises.
I understand that being in the performing arts, Boogaloo Academy Ltd./A Star Society provides many opportunities for videos and photos to be taken. As such, I permit any such videos and/or photographs to be taken and used by Boogaloo Academy Ltd./A Star Society, in any manner, including for promotion and flyers.
I agree to receiving e-mails from Boogaloo Academy/A Star Society.
I acknowledge and agree that all fees paid to Boogaloo Academy Ltd./A Star Society are non-refundable for any reason, including without limitation, to studio closures that are beyond the control of Boogaloo Academy Ltd./A Star Society, in which case credits or make up classes will be provided.
My agreement below confirms that I understand, agree and accept the foregoing and I give my consent as Participant, or, I give my consent for Participant as the Parent or Legal Guardian, to participate.
